What is the pre-operative preparation like for Gastric Banding surgery?

Like all major surgeries, pre-operative preparation for Gastric Banding consists of screening studies like gastroscopy, routine blood examination, and a thorough consultation with physical examination by physician. Make sure that patient inform surgeon about his other health condition, all the prescription and non-prescription medications including vitamin, mineral and herbal supplements that patient take. Patient will be instructed to stop smoking at least 2 weeks before his Gastric Banding surgery and not smoke until the stitches are completely healed. Patient should try to reduce calorie intake and lose as much weight as possible before the Gastric Banding surgery. Patient will be instructed not to eat or drink any thing for at least 12 hours before the Gastric Banding operation. Patient may be required to get hospitalized day before or morning of Gastric Banding surgery.

What are the variants of Gastric Banding surgery?

The type of Gastric Banding procedure can be performed either by using a band, a ring, staples or a combination of all.

1.Open Gastric Banding – The band is placed around the upper part of the stomach, forming a small gastric pouch to limit food intake and slow the food passing from the stomach into the intestines.

2.Laparoscopic Gastric Banding – This procedure is popularly known as Lap. Band or Adjustable Gastric Banding. Laparoscopic Gastric Banding is performed with the help of a Laparoscope. In this surgery a small pouch is created by placing a band around a portion of the stomach, thus reducing it’s size considerably. The band can be adjusted to accomodate the increased need for food e.g. during pregnancy.

3.Vertical Banded Gastroplasty (VBG) – This procedure of Gastric Banding uses a combination of staples and band. In this procedure, staples are used to create a small pouch, which limits the amount of food that can be eaten at one time. The band then prevents the food from leaving the pouch too quickly thus giving patient a full feeling that lasts longer.

4.Silastic Ring Gastroplasty (SRG) – In this procedure, a silastic ring is placed at the stoma to control its size and prevent it from over stretching.

The Surgical Procedure:

If gastric band surgery is done laparoscopically, surgeon will make several small incisions (each about an inch long) in various places on abdomen. Tubes will be put into these small incisions to provide a passage into abdomen.

One tube will have a very tiny camera through which surgeon can see inside patient’s abdomen on a video monitor. Others will allow the surgeon to place the gastric band around the stomach. If any difficulties arise, surgeon may use an open procedure. In this case, he or she will make a vertical incision about eight inches long in the abdomen to expose stomach.

During the gastric band surgery, surgeon will place the band around stomach to create a narrow opening between the upper and lower parts of the stomach. Another tube with an inflatable balloon will be passed into patient’s mouth, esophagus and stomach to allow the surgeon to make the upper pouch the correct size. When it is, the surgeon attaches the band and secures it in place by sewing parts of the two sections of stomach over the band. Then the tube and balloon are removed from esophagus and stomach. The surgeon will implant the reservoir port just below the rib cage.

Patients are usually able to leave the hospital the day after gastric band surgery, but this depends on surgeon’s recommendation about patient’s specific case. Patient should return to surgeon for routine postoperative care about a week after patient leaves the hospital. Also at that time patient should enroll in the Weight Management Program.

When patient get home, patient should walk as much as is comfortable for patient. Light housework, driving and other daily activities can usually begin five days after surgery. Patient should avoid heavy exercise and limit lifting to 20 pounds or less. Although some patients are able to go back to sedentary jobs within a week after surgery, it is more usual to wait two to three weeks.

Bar Chords – What’s the Easiest Way to Play Bar Chords on the Guitar?

Bar chords are the first advanced step for the guitar player. You have some kind of understanding of how to play open guitar chords and you have practiced till your fingertips are hardened enough not to hurt any more. So the next step is to learn the basic open chord shapes that move up the guitar neck to make bar chords and to start on the next stage of educating your left hand and arm in the art of guitar playing.

When you learn bar chords you will see that there are some basic chords that you have already learnt how to play that can be moved up the fretboard to form chords in more keys. The way we move open chords up the fretboard is by placing the first finger on the left hand across the guitar’s neck and using the other fingers to complete the chord shape.

When you first try to play bar chords you will notice the change in sound. You have made all the guitar strings shorter, so they do not have the same amount of sustain and reverberation. The next thing you notice is that bar chords are painful. The pain should lessen as you practice but if it doesn’t there are some things about your guitar playing you will need to look at.

First, if there is any adjustment needed to the way you press down on the strings with your left hand, you will need to relax your hand and experiment a little. Hold the chord shape but do not exert the pressure needed to make a clean sound. Now see if there is any tension in your arm. Just check to see if you can feel it. Go all the way up and down the arm fro the shoulder to the fingertips and relax any little areas of tension. This might result in some change in how you are holding your hand on the strings. As you try this checking for tension, also take note of if you hold your breath and clench your jaw. If you do either of those things, let the tension drop away in those areas.

Other areas that might need attention when you are looking for the easiest way to play bar chords are the height of your guitar’s action and the need for hand exercises. You can do an online search for balls or other gizmos that you can use to help build finger strength, or for hand and finger strengthening exercises. But before you get too far into exercises, if your guitar’s action is high you will need to press hard to make bar chords. Acoustic guitars have a relatively high action so the strings do not buzz. If you feel that your action might be too high you can adjust it by filing down the slots in your bridge so that the strings sit lower. If you make the strings too low you will need to buy another bridge bone.

If you are able to switch between acoustic and electric guitars you can clear up the question about your guitar’s action because electric guitars have a lower action than acoustics. There are also tricks that you can use to avoid the pain of guitar chords, like using the open D fingering to move up the frets instead of using the open E shape to make a bar chord. This limits your chords to the top three strings. Also you can try loosening the tension on your strings or fooling around with open tunings, but do not go meddling with your guitar unless you are really sure you have some medical condition that stops you from playing bar chords. For now just assume that your hand will stop hurting once you have practiced enough.

Is £6 a one hour session a reasonable price for a kickboxing class?

I’ve been doing a kickboxing class these last few weeks, and have enjoyed it. It’s been £5 a session up til now, but now they want me to sign up with a direct debit of £24 a month, and also pay a £50 license/insurance fee, and buy a ‘uniform’. This seems like a lot of expense for 1 hours training a week. Is this normal?
Also, I think 1 hour a week isn’t enough, do kickboxing training gyms exist with punch bags etc, and how much do they charge?
£6 is equivalent to about $12 by the way.

Is there a chin up bar available that doesn’t damage the door frame and that fits walls thicker than 7 inches?

As stated above, I’m looking for a chin up bar that would fit a door frame and not leave any damage, as I rent my place.

I’ve tried the IronGym but my walls and door frame were too thick (IronGym only fits between 4.5 – 6.5 inches). The PowerBar also suffers from the same problem. Permanent fixtures are not an option.

Lap Band Surgery – Know Your Options

Did you know that unlike gastric bypass surgery, the adjustable gastric band or lap band surgery procedure is reversible? The LAP-BAND System received approval from the Food and Drug Administration for use in the United States back in June 2001. Another type of gastric band, the Swedish adjustable gastric band, differs from the LAP-BAND because it’s not designed for use with keyhole laparoscopic surgery.

In general, gastric banding, including the Lap Band procedure and weight loss system is indicated for people for whom: Body Mass Index is above 40, or for those who are 100 pounds (45 kg) or more over their estimated ideal weight according to the 1983 Metropolitan Life Insurance Tables or for those between 30 to 40 with co-morbidities, which may improve with weight loss (high blood pressure, diabetes, sleep apnea, and arthritis). Mentally disabled or emotionally unstable people won’t be considered for lap band surgery. The lap band surgery is usually not performed if one has an untreated glandular disease like hypothyroidism, where other solutions may still be sought.

In order to be considered for surgery one must have the comprehension of the risks and benefits of the gastric band procedure and willingness to comply with the substantial lifelong dietary restrictions required for long term success. Failure of dietary or weight-loss drug therapy for more than one year can also make one eligible for the adjustable gastric band procedure.

Gastric dumping syndrome problems that occur in other types of surgeries do not occur in lap band surgeries because no intestines are removed or re-routed. Unlike those who have procedures such as Roux -en -y gastric bypass surgery, duodenal switch or biliopancreatic diversion, it’s unusual for gastric band patients to experience nutritional deficiencies or malabsorption of any micronutrients. Another plus is the surgeon uses a specialized needle to avoid damage to the port membrane.

Lap band or gastric band placement, unlike the traditional malabsorptive weight loss surgeries does not cut or remove any part of the digestive system. The placement of the adjustable gastric band creates a small pouch at the top of the stomach, which holds approximately 50 ml. This pouch ‘fills’ with food quickly and the passage of food from the top to the bottom of the stomach is slowed down. Calcium supplements and Vitamin B12 injections are not usually required following gastric banding as they are with Roux-en–y gastric bypass surgery and other surgeries.

The amount of weight that is lost following the months after surgery will depend on the individual and their personal circumstances, motivation, and mobility. Some patients may find that before their first ‘fill’ appointment they are still able to eat fairly large portions. Post surgery the patient should consider eating less, eating slowly and chewing food thoroughly.

Following surgery, adjustments, which are also called ‘fills’, may be performed using a fluoroscope so the radiologist may evaluate the placement of the band, the port and the tubing, which runs between the port and the band. Initial weight loss in gastric banding is slower than with Roux-en-y gastric bypass surgery but statistics indicate that over a five year period the weight loss outcome is usually similar.

Complications of gastric band surgery include ulceration and irritated stomach tissue. Some of the mechanical malfunctions that can occur post-surgery are port leakage, cracking of the kink-resistant tubing or disruption of the tubing connection from the port to the band, port site pain and port displacement.

Some of the adjustable gastric band surgery complications involving the band itself and port can include band slippage, pouch dilation, esophageal dilatation or dysmotility and erosion of the band into the gastric lumen. Erosion can occur where the band may wear a small area on the outside of the stomach which can lead to migration of the band to the inside of the stomach.

Many factors can affect the total cost of surgery, including the gastric surgeon you choose, where you choose to have the surgery performed, and what fees are included or not included in the price and if you’re traveling out of the country. Some post-surgical weight loss surgery teams offer support groups, but some of them mix gastric bypass surgery patients with gastric banding patients, so it’s important to try to find a support group for gastric banding only. The adjustable gastric band surgery is fully reversible, has a short hospital stay and there is usually a quick recovery, but be fully aware of the complications that can occur.
